Specialised Case Management Programme (SCMP)
People who acquired disabilities due to illnesses (such as stroke and diabetes) and traumatic injuries (such as spinal cord injury and head injury) need to deal with the anguish of disability, grief and loss. Some may need help to adjust to living with a disability. Those with the potential to engage in employment require training and development of new skills.
Caregivers, who are the primary informal resource for people with disabilities, also need to be supported, as caring for a loved one with a disability can get stressful. Similarly, the family equilibrium may be thrown into a state of instability where roles, functions and expectations within the family need to change in order to accommodate the needs of the disabled person.
The Specialised Case Management Programme is a nation-wide pilot programme which started in October 2006. This programme caters to persons of all age groups with disabilities, as well as to their caregivers, to empower them to lead more fulfilling and wholesome lives.
If you have a disability, we will help you to:
- Learn ways of adjusting to life challenges
- Cope with grief and loss following the disability
- Develop appropriate social, cognitive and behavioral skills
- Make responsible and effective choices for home, work and the future
If you are a caregiver, we will help you to:
- Learn ways of coping with the responsibility of looking after and caring for a loved one with a disability
- Find support and help for problems and complex needs in the family
The services we offer are:
- Case Management – to coordinate and acquire resources to support the disabled person and his/her family
- Counselling
- Consultancy on disability information and issues
- Group ⁄ Peer Support
- Vocational ⁄ Motivational Counseling and Coaching
- Psychoeducational workshops to provide information on health issues and knowledge of self-care
- Caregiver Support and Training
- Wellness Programmes for people with disabilities and their caregivers to enhance their physical health and psychological functioning, such as fitness classes.
