Walk-in registration at SPD Centres (Payment via cash only)

1. Download or obtain a registration form from any of the following SPD centres
2. Complete the form
3. Submit the form and make payment at any of the following SPD centres:
   • SPD HQ
     2 Peng Nguan Street, SPD Ability Centre, Singapore 168955
     Operating hours are from 8:30am to 6:00pm, Mondays to Fridays
   • SPD Jurong
     Blk 337, Jurong East Ave 1 #01-1562 Singapore 600337
     Operating hours are from 8:00am to 5:30pm, Mondays to Fridays
   • SPD Bedok
     Blk 522 Bedok North Ave 1 #01-312 Singapore 460522
     Operating hours are from 8:00am to 5:30pm, Mondays to Fridays
   • SPD Tampines

     Operating hours are from 8:30am to 6:00pm, Mondays to Fridays
