The Integrated Community Care Provider (ICCP) acts as a one-stop, neighborhood-centric touchpoint for senior care. Part of the Age Well SG initiative, it connects seniors to Active Ageing Centres (AACs), Rehabilitation, Day Care, and Enhanced Home Personal Care services, ensuring seamless care for seniors to age in place safely and actively.

Active Ageing Centre (AAC)
Activities include but not limited to exercise, dance, art & crafts, music and cooking classes, outings, health talks etc.
